The numbers on your laser’s display describe what the device emitted. They do not tell you what happened in the skin. The whitening that appears immediately after the pulse — the frost effect — is the real indicator of how intensely you have acted on the pigment, and reading it correctly is the most important practical skill in this work.

Why skin turns white

The pulse is absorbed by a pigment particle, which explodes. The energy released at that moment instantly vaporises intracellular and intercellular fluid, forming microscopic gas bubbles — oxygen, carbon dioxide, nitrogen, vapour — through the dermis and epidermis. Those bubbles scatter light, and scattered light reads as white.

Think of opening a bottle of sparkling water. The gas was dissolved and invisible; the moment the pressure changes, bubbles appear everywhere at once. That is what you are looking at on the skin.

Five grades of reaction

Direct tissue destruction with bleeding — unacceptable

Caused by too small a spot diameter combined with excessive fluence, producing optical breakdown and mechanical destruction of tissue. You will see bleeding and an obvious skin defect immediately after the flash. The risk of scarring and pigmentation disorder is high. Stop at once — this is not a setting to adjust, it is a way of working that has to end.

Hyperfrost — dangerous

A small spot at high fluence over abundant pigment. The skin does not merely whiten — it visibly swells and increases in volume. So many bubbles form that pressure inside the tissue rises and microcirculation is compromised. The probability of blisters and burns is very high. Reduce the intensity: increase the spot diameter, lower the fluence. Treat it as an alarm, not as a strong result.

Moderate frost — the target

Even whitening, no swelling, no bleeding. The intensity matches the amount of pigment present. Frost is most pronounced across the first one to three sessions, and the risks stay manageable provided cooling afterwards is done properly.

Weak frost, or only redness and oedema

By the fourth session and beyond there is simply less pigment left, so frost becomes faint or disappears. This is normal — the laser is still working. Judge the effect by the oedema three to five minutes after treatment instead.

No reaction at all

Professional ink with very fine particles against insufficient peak power from a compact laser. The device is having minimal effect at these parameters, and progress will be very slow even with long intervals between sessions. This is an equipment conversation, not a technique one.

Frost is a judgement, not a number

“Moderate frost” is not an absolute value you can calibrate to. It is a range that you learn to assess by seeing many examples of different reactions. And the balance never changes: the more intense the frost, the higher the efficiency and the higher the risk.

If you are early in this work, aim deliberately for moderate and rather weak frost. Working close to the upper boundary is something you earn with clinical experience, not something you start with.

Test pulses: the protocol

Mandatory for every new patient and every difficult case:

  1. Set your intended working parameters
  2. Deliver one or two test pulses onto the tattoo or PMU area
  3. Assess the degree of frost
  4. If in any doubt, wait 10–15 minutes
  5. Confirm there are no pathological reactions — no wounds, bleeding or blisters
  6. Only then proceed to the full treatment

Two minutes spent checking is cheaper than a complication. That trade has never once gone the other way in my practice.

Oedema as the late indicator

Three to five minutes after treatment, the degree of swelling reflects the total intensity you delivered. On later sessions, where visible frost is absent, this becomes your main reading. Pronounced oedema means the intensity was high — which means cooling now deserves your full attention.
